Making critical health-care connections to lower hospital readmission rates

Heart failure is one of the most common reasons for hospital readmission in Canada. In an effort to better support patients and alleviate the strain on the health-care system, a Trent University student’s research is helping identify interventions that may lower readmission rates in the future.

Through community-based research with the Peterborough Regional Health Centre and a local cardiologist, the student is comparing pre-pandemic and present-day medical chart data to assess variations in demographics and medical history among heart failure patients.

Their research will also explore how the pivot to primarily virtual health care during the pandemic changed patterns in hospital readmission.
